Passport Information

To submit a passport application, applicants must make
an appointment with the Clerk’s Office. 

Passport applications will be accepted
Monday – Thursday no later than 2:30 pm
Friday no later than 11:30 am

** If children under the age of 16 both parents must be present **

ITEMS TO BRING WITH YOU

  • Unsigned DS-11 Application
  • 2 – Identical Passport photos
    • must be 2″ x 2″ in size
    • must have a white or off-white background
    • Use a clear image of your face
    • No eyeglasses
    • No selfies
  • Original – Certified U.S. Birth certificate or naturalization papers
  • Black and white copy of Original Certified U.S. Birth certificate or naturalization papers
  • Original State issued ID: Drivers License or ID card.  Military ID may be used
  • Black and white copy of State issued ID
    • Copy front and back on single side of paper
  • Required fees
    • Check or money order made payable to: U.S. Department of State
